Pledge of Allegiance

  1. Call to order.

The meeting was called to order at 9:10 a.m. Present was Banaszak, Mueller, Sotka and Tarmann.

  1. Approval of agenda.

A motion to approve the agenda was made by Sotka, seconded by Banaszak and carried.

  1. Review health and dental insurance plans from State of Wisconsin

Information was presented on the resolution to join the WI Public Employers’ Group Health Ins. Program, including the 4 progam options of medical insurance with dental and 4 program options of medical insurance without dental. The resolution requires action by the Town Board to be accepted into ETF Group. The Town Board agreed upon program option #3 Coinsurance HMO Standard PPO w/Dental, P07

  1. Resolution for Inclusion under the WI Public Employers’ Group Health Ins. Program  (see attached resolution)

Roll call vote:A roll call vote taken: Banaszak yes, Mueller yes and Sotka yes, and carried.

  1. Employee Handbook updates concerning insurance and probation

The Employee Handbook was reviewed to include changes to #11 Insurance. The Town of lake Board shall pay %80 of the total premium of the dental and health insurance plan, and the employee shall pay %20 of the total premium of the dental and health insurance plan as of August 25, 2019.

Changes to #2 to include #2(a) Probationary Period: The probationary period for new employees hired by the Town of Lake Board shall be a probationary period of 90 calendar days from the first day worked.

  1. Closed session: Pursuant to Wis. Stats. 19.85 (1) (a) Personnel. The Board may  reconvene to open session to take any action.

Roll call vote: Mueller yes, Sotka yes, Banaszak yes and carried at 9:57 a.m.

  1. Reconvene into open session
  2. Action: The Town Board reconvened into open session at 10:30 a.m. A motion was made to offer employment to candidate #1 with insurance at the pay rate of $20.00 as approved in the previous motion with changes in the handbook. Sotka made the motion, Banaszak seconded the motion with 2yes and Mueller voting no, and carried.

A motion to adjourn was made by Mueller, seconded by Sotka and carried at 10:37 a.m
